John BENNETT died on the 07th May 2022 at the St Catherine's Hospice. A post-mortem concluded Mrs GOWERS died from sepsis as a result of suppurative osteomyelitis as a consequence of the sacral pressure sores However, Mrs GOWERS deteriorated despite involvement of tissue viability nurses and was readmitted to hospital where she did not recover. As a result, district nurses attended at the care home to treat the sacral pressure sores. She was discharged to a care home where sacral pressure sores were noted. Following a fall Mrs GOWERS was admitted to hospital where a fractured neck of femur was diagnosed and operated upon. Kathleen GOWERS died on 18 January 2022 at Royal Blackburn Hospital, Blackburn. Mr John William CLARK was admitted to Royal Blackburn Hospital where he died on the 22nd March 2020 from multi organ failure contributed to by necessary heart bypass surgery undertaken on the 22nd January 2019. Lilliana's death was contributed to by not intervening sooner to expedite the birth by Caesarean Section not having been undertaken at an earlier stage when required, which amounted to neglect. During a difficult breech birth clinicians did not agree a clear plan when Lilliana's birth required intervention. Prior to her birth Lilliana was identified to be lying in a breech position which she remained in.
However, she showed little signs of recovery and did not survive. Following extensive resuscitation, she showed signs of severe encephalopathy and was commenced on hypothermia therapy. Lilliana was born on 5 November 2020 showing no signs of life. Lilliana TAILOR died on 7 November 2020 at Royal Preston Hospital, Preston in Lancashire.
